lost close button options

hi guys (first time here)

seem have lost 'close' & 'minimise' buttons on tabs????
please? i've trawled through setups.

 

what application talking or mean red, yellow , green traffic lights on top left of every window of every application?
happen in another, temporarily created, user account?
