Volume stuck on maximum level, keys work but volume doesn't change

i've had problem before, simple restart has solved problem in past. system volume @ maximum. appears can control volume volume keys on keyboard or in sys prefs, when volume slider way @ bottom, still sounds it's @ max. volume control work when output internal speakers (from mac pro), not when output led cinema display.
