Assign note to a contact so it appears when called?

is there way, or app, allow me see note when contact calls me, or call contact? having contact's note filed pop on screen when contact calls good.

you take screenshot of note (press home , sleep/wake together) , attach picture contact. then, when contact calls you, (or call contact), screenshot shown.
