Baffling Visual Error

so upon resetting ipad 1 afternoon came across this.


noticed persistent after reset , move other apps across home screen when in wiggle mode. can foldered once dropped in folder become default newsstand app. not jailbroken , news stand not in folder begin with.

else experienced this?

 

i don't notice wrong. missing?
