iPhone Forgets my number

for reason phone keeps forgetting number. started happening couple days ago. happens when go send imessage. message go setting , enter number. click on , have input number.

tried searching , had no luck. else have happening or have suggestions on how fix issue.

have 4s running 5.1

 

i'm getting error. same thing, go reply imessage , prompt need enter in phone number.
