Handbrake .m4v?

hi,
i'm having issue handbrake @ moment.

presets appear fine far i'm aware, work perfect. example, have ripped family guy season 5 , fine, plays fine , everything.

however, when morning tried rip casino royal (film - have done other films before worked fine) after file had finished, when tried open it said in quicktime "this movie not opened, file not movie file" though .m4v.

i've tried in vlc , literally vlc crashes!

help?

 

change extension .m4v .mp4 manually. "are sure want change extension?" yes.

handbrake uses .m4v instead of .mp4...i change .mp4 before adding handbrake que manually.
