Any Customized UI for UITextField and UILabel?

here ui ios looking for:

in view of application ios, user asked fill out underline blanks in not fixed number of distinct questions of following form(for example):

id _ _ _ _ , telephone number _ _ _ - _ _ _ _.

since questions not fixed, uitext not enough use requirement. spent 3 days on finding proper ui control , view did not find it.

there ui control , view requirement? if not, need build own ui control , view using uikit. recommend reference building customized ui using uikit? suggestion welcomed. thank you.

 

what considering uitableview variable number of rows, each cell containing uilabel , uitextfield, similar edit mode in contacts app?
